Manage complexity without starting over
You're seen completely: your history, your patterns, your goals, not just today's symptoms. Care that accounts for the complexity of juggling responsibilities while respecting your time.
Virtual-only, membership-based primary care for Washington State patients, built for continuity, grounded in your biological baseline, and designed to make care more informed with every visit.
Waitlist is free and creates no obligation. Joining does not create a doctor-patient relationship or guarantee enrollment. Washington State residents age 12 and older only. Not for emergencies; call 911 for immediate medical needs.

Good medicine takes time to listen, review what came before, and think carefully about what comes next.
Sankofa Family Medicine was built for continuity: a deliberately small panel, the same doctor every visit, and the time to know each patient deeply. From the start, your physician works to build a foundational picture of your health, drawing on your history, genetics, and labs, and uses that context to support more informed decisions as your care evolves, year after year.
This practice is for people who want to be known deeply, consistently, over time. Whether you are building a career, raising a family, or managing your health for the long term, this was designed to work with your life, not against it.

You're seen completely: your history, your patterns, your goals, not just today's symptoms. Care that accounts for the complexity of juggling responsibilities while respecting your time.

The same doctor for your whole household, visit after visit. Your doctor works to build a baseline for each family member and understands how individual health patterns relate, so care is connected in context, not just by name.

You stay ahead. Early in your care, your physician works to establish a foundational baseline, including labs and, when clinically appropriate, genetic information, so future decisions are supported by real context. When something changes, your doctor interprets it against what they already know about you, not in isolation.

Your schedule matters. Virtual-first care designed to fit your life, not the other way around, without sacrificing depth or continuity.
From first click to first visit, here is exactly what to expect.
Sign up at no cost. You will receive updates as we approach enrollment. No commitment, no payment, and no clinical relationship until you choose to move forward.
As enrollment approaches, you will receive a short questionnaire about your health priorities and what you are looking for in a physician. This helps us determine if the practice is a good fit for your needs.
If the questionnaire suggests a good match, we will invite you to a brief introductory call so you can meet your physician, ask questions, and decide if the practice is right for you.
Accepted members begin the Foundation Period. Over the first several months, we build a thorough understanding of your health through intake, labs, and a comprehensive first visit. This is how your physician gets to know you deeply.
After your baseline is established, your care settles into a steady rhythm. Regular visits, secure messaging, and a physician who knows more about you with every conversation.
Four qualities that define every membership, every visit.
Visits are designed so you can think, ask questions, and understand your options clearly.
The same doctor carries your history forward. Every visit builds on the last, with a deliberately small patient panel so nothing gets lost.
Messaging and scheduling support continuity, with clear expectations, clinical prioritization, and a transparent membership structure.
Your doctor works to build your baseline once, using foundational labs and, when clinically appropriate, genetic information, and uses that context throughout your care. When a clinical question arises, the answer is shaped by your history, your biology, and your response patterns. That is what precision-informed medicine means here: context applied to judgment.
Precision-informed medicine is not constant testing or algorithm-driven care. It means your physician uses a complete, established picture of your health, including your history, baseline labs, and genetics, to support more accurate decisions when it matters.
| Step | What Happens | Why It Matters |
|---|---|---|
| Build the baseline | Foundational labs and, when clinically appropriate, genetic information are established early in the relationship | Your doctor has real biological context before a problem develops |
| Carry it forward | Every visit, every result, and every decision is read against what your doctor already knows | Care builds instead of resetting |
| Apply precision when needed | When a clinical question calls for deeper analysis, tools are used purposefully and at physician discretion | Judgment is informed; technology supports the physician, not the other way around |
Continuity is the foundation. Precision is applied when it supports the decision in front of us.

Whether you are managing a chronic condition, recovering from a health scare, or simply want a practice that helps you stay ahead, this was designed for people who want more from their primary care.
If this sounds right for you, the Founders Waitlist is open. We'll be in touch when enrollment begins.
Your membership is built around one thing: giving your doctor the time and structure to actually know you. Details are shared when we contact you from the waitlist.
When you join, we begin building your baseline. Foundational labs and a genetics panel are included from the start, not billed as add-ons, so your physician has a real biological picture of you before problems arise. As care continues, your doctor uses that foundation to interpret new results, support clinical decisions, and adjust your care based on how your body responds over time. These tools are advisory and support physician judgment; they do not replace the relationship.
