Sign up at no cost. You will receive updates as we approach enrollment. No commitment, no payment, and no clinical relationship until you choose to move forward.

Complete a Brief Questionnaire

As enrollment approaches, you will receive a short questionnaire about your health priorities and what you are looking for in a physician. This helps us determine if the practice is a good fit for your needs.

Talk with Your Doctor

If the questionnaire suggests a good match, we will invite you to a brief introductory call so you can meet your physician, ask questions, and decide if the practice is right for you.

Enroll and Build Your Baseline

Accepted members begin the Foundation Period. Over the first several months, we build a thorough understanding of your health through intake, labs, and a comprehensive first visit. This is how your physician gets to know you deeply.

How Membership Works

Your membership is built around one thing: giving your doctor the time and structure to actually know you. Details are shared when we contact you from the waitlist.

When you join, we begin building your baseline. Foundational labs and a genetics panel are included from the start, not billed as add-ons, so your physician has a real biological picture of you before problems arise. As care continues, your doctor uses that foundation to interpret new results, support clinical decisions, and adjust your care based on how your body responds over time. These tools are advisory and support physician judgment; they do not replace the relationship.

Does Sankofa Family Medicine accept insurance?

No. This is a self-pay membership practice. We do not bill insurance for membership services. If you have an HSA or FSA, those funds may be applicable, subject to your plan rules. We can provide a superbill upon request when clinically appropriate.

What does virtual-only mean for my care?

All visits take place via secure, HIPAA-compliant video. No special software is required. You can join from any device. You will always see the same doctor, and your visit will be designed for depth, not speed.

Is the Founders Waitlist a commitment to enroll?

No. Joining the waitlist is free and creates no obligation. It does not establish a doctor-patient relationship or guarantee enrollment. When enrollment opens, you will receive details on membership options and rates before making any decision.

Who is eligible to join?

Sankofa Family Medicine currently serves Washington State residents age 12 and older. All care is delivered via telehealth.
