About Our Practice

Your Story Stays With You

A virtual-only, physician-led, membership-based primary care practice built on continuity. The same doctor carries your history forward so care compounds instead of resets - visit after visit, year after year.

What does Sankofa mean?

Sankofa is an Akan principle from West Africa, symbolized by a bird looking backward while moving forward. It conveys that it is right and necessary to return to the past, retrieve what was learned, and carry it forward for a higher purpose.

In healthcare, this principle highlights a common challenge. Medical records often fragment across systems, doctors often change between visits, and patients are frequently asked to repeat their history when continuity is not preserved.

Sankofa Family Medicine was designed for continuity. The same doctor who knows your history, visit after visit, so medical decisions build instead of reset.

Our Philosophy

How Precision-Informed Care Works Here

Primary care built on continuity requires understanding more than today's complaint. It requires knowing your history, how your biology behaves, and how past decisions shaped where you are now.

At Sankofa, your physician works to build a foundational picture of your health early, drawing on your history, labs, and genetics, and carries that forward across every visit. When a clinical question calls for deeper analysis, that baseline becomes the lens through which it is answered. Precision is not a product here; it is how your doctor thinks.

Genetics-Informed Care

Your genetic information is established once, early in care, and serves as a permanent reference layer. It informs how your physician interprets labs, considers medication responses, and personalizes prevention recommendations, as clinically appropriate. It does not drive every decision, but when a decision matters, it helps ensure your doctor has biological context, not just a clinical snapshot.

Continuity of Care

The same doctor who knows the depth of your history across every visit, so care builds instead of resets.

Context Preserved

Your doctor does not just see a list of medications. They know why you started them, how your body responded, and what that response means for your care going forward.

How It Works

Membership-Based Primary Care

Founded in 2025, operating from Spokane, Washington, and serving patients age 12 and older statewide through telehealth.

  1. Join the Founders Waitlist

Secure your position as a Founding Member with enrollment priority. No payment required.

  2. Complete Secure Onboarding

When enrollment opens, complete intake in our HIPAA-compliant clinical platform. Share your medical history, current medications, and health goals so your doctor can begin building the longitudinal understanding that defines this practice.

  3. Begin Your Care Partnership

Start receiving comprehensive virtual primary care visits with direct access to your doctor. Our platform preserves the details that matter, so your care is informed by who you are, not just what brought you in today.

"We begin with your story, not your chart."

Our Commitments to You

We listen first

Your story matters as much as any test result. What you have lived through shapes how we approach your care.

We document with dignity

We write your records about you as a person, not a billing code. We document your care with the precision and respect it deserves.

Full transparency

Clear explanations, access to your records, and transparent communication about the self-pay membership model.

Fair treatment

Your background informs care. It never limits the quality of care you receive.

Your questions, taken seriously

Secure messaging responses typically within 24 to 48 hours for established patients, subject to clinical circumstances.
