About Dr. Nkrumah

Dr. Yaw Nkrumah is a board-certified family doctor and the founder of Sankofa Family Medicine.

He founded a membership-based primary care practice built on continuity-centered care, with physician-led interpretation of genetic health information and structured tools when clinically appropriate. His approach emphasizes relationship-based family medicine, prevention, and careful follow-through over time.

He trained at the Medical University of South Carolina and completed his Family Medicine residency in Washington State. During training, he saw how often care breaks down when doctors change and history gets lost.

Sankofa Family Medicine was created to fix that problem. The goal is simple: your doctor who knows you, your relationship that builds over time.

Currently, Dr. Nkrumah serves as the sole physician. The practice is designed to maintain a small patient panel and continuity-first model.
